The collision happened in Green Lane, Whitgreave, near Stafford, at about 1830 BST on Wednesday.
West Midlands Ambulance Service said they found the child semi-conscious with multiple serious injuries.
He was transferred to hospital on a spinal board where his injuries were judged to be life threatening. The van driver was not seriously hurt.
