Medicare drug plans generally provide coverage for valganciclovir (Valcyte) or a similar antiviral medication for people with stage 3 HIV.
Valganciclovir (Valcyte) is an antiviral medication available as a tablet or oral solution that doctors prescribe to treat conditions linked to stage 3 HIV (commonly referred to as AIDS), such as cytomegalovirus (CMV) eye disease and pneumonia.
Medicare Part D and Medicare Advantage Prescription Drug (MAPD) plans have formularies (lists of covered drugs). These lists must include at least two drugs in each of the most commonly prescribed categories of medications.
Additionally, these plans must cover most medications within six protected drug categories. This means that many Medicare drug plan formularies should include valganciclovir.
That said, private insurers administer these plans, so coverage varies by plan. If a plan’s formulary does not include this drug, Medicare drug plans should cover a comparable alternative.
While valganciclovir does not cure illnesses associated with stage 3 HIV, it can help slow their progression and reduce symptoms.

A bottle of generic valganciclovir solution can typically cost between $89 and $220, while the brand-name drug Valcyte costs about $1,100.
Medicare drug plans use tiered pricing, where higher tiered drugs require larger copayments and generics usually fall into lower, cheaper tiers. The tier valganciclovir falls into depends on the plan.
However, in 2025, Medicare capped annual prescription costs at $2,000, increasing to $2,100 in 2026. This means Medicare covers all remaining costs after a person reaches that spending limit.
In addition, Part D and MAPD plans both charge monthly premiums, which also depend on the plan. MAPD enrollees must also pay the Part B premium, though some Part C plans may reimburse part of or all of this cost. In 2025, Medicare drug deductibles cannot exceed $590.
For additional help with valganciclovir costs, a person may turn to programs like Medicine Assistance Tool (MAT) and NeedyMeds, Medicaid (if dually eligible), the Ryan White HIV/AIDS Program (RWHAP), the HRSA Health Center Program, or programs for veterans and American Indian and Alaska Native populations.
Drug manufacturers and independent organizations may also provide free or reduced-cost drugs, and the Extra Help program, also known as the Part D Low Income Subsidy (LIS), can assist those with limited income.
